The documentation below is required to participate in the football or cheer programs. A BOYFL participation form will be available at registration. This includes contact information, payment detail, and confirmation that you have read and agreed to abide by our 2012 Code Of Conduct. Copies of these documents will be available at registration. All other forms are due before the first practice of the season, which is typically August 1st.
2012 Pop Warner Player Participation Contract (grade and school information should be for the 2012/2013 school year)

2x3 photo (a wallet size photo; no hat or sunglasses per Pop Warner rules)
2011/2012 year end report card
Registration costs are $325 for football and $225 for cheer. Cheer registration does not include the cost of the uniform.
